Markersuche bei gesicherter B-CLL. Gemäß umfassender Literatur (s.Anm.) kann sich - insbesondere zur Optimierung der Therapiesteuerung vor einer geplanten Erstlinientherapie von CLL mit hohem oder sehr hohem Risiko bzw. Zweitlinientherapie refraktärer Patienten, zur möglichen Erkennung weiterer Patienten mit einem solchen Risiko (IPI unabhängig) z.B. bei jungen/fitten Patienten zum Zeitpunkt ED - diese erweiterte Mutationsanalyse prognostisch und therapeutisch relevanter Genloci anbieten.

Die Bestimmung des IgVH Mutationsstatus ist zusätzlich zu empfehlen.

Indication

Bei ansteigender Resterkrankung unter Therapie mit Tyrosinkinaseinhibitoren (z.B. Imatinib/ Dasatinib/ Nilotinib/ Ponatinib/ Bosutinib), bestätigtem Verlust einer majoren molekularen Remission und nach jedem Wechsel des TKI sollte die Sequenzierung der ABL-Kinasedomäne von BCR-ABL erfolgen. Ziel ist ein rechtzeitiges Erkennen einer meist sekundären Resistenz zwecks gezielter, therapeutischer Intervention (z.B. Wechsel des TKI, Suche nach Knochenmarksspender, Wechsel auf andere Präparate).

Indication

Zur Differentialdiagnose und weiteren Verlaufskontrolle der CBFß-MYH11 positiven AML FAB M4eo.
Zur Differentialdiagnose bei Hämoblastosen, ALL, CML, AML.
Eine 16q22 Anomalie ist nahezu pathognomonisch für AML des FAB Subtyps M4eo, tritt aber sehr selten auch bei AML -M2 -M5 oder der -M4 ohne Eosinophilie auf, außerdem bei MDS und CML in Blastenkrise.

Positive Proben können zusätzlich auf cKIT-Mutationen der Exons 8 und 17 geprüft werden (KIT mutiert mit höherer Rezidivrate, jedoch ohne Einfluß auf das OS, ggf. therapierelevant).
Etwa 30% der AML M4 und 20-25% der AML M2 weisen ebenfalls Mutationen des Gens KIT auf. Diese verschlechtern die ansonsten gute Prognose (höheres Rezidiv-Risiko M2+M4, niedrigeres Gesamtüberleben M2). Vorliegende KIT Mutationen können als therapeutische Targets genutzt werden. Hierbei ist die genaue Identifikation der vorliegenden Mutation überaus relevant für die Therapiewahl!

Indication

Rekurrente Mutationen von CSF3R finden sich bei 35-83% der CNL und seltener bei aCML (3.3%) und CMML (0.8%).
Neue Therapieoption der CSF3R positiven CNL mit membranproximaler ligandenunabhängiger Aktivierung wie z.B. bei p.Thr618Ile, ist u.a. der JAK Inhibitor Ruxolitinib.

Note

Differentialdiagnose zwischen aCML und CNL ist nur anhand hämatologischer Parameter möglich. Auf bei aCML vs. CNL relativ häufigere Mutationen von SETBP1 kann ebenfalls untersucht werden. Geeignetes Panel z.B. CSF3R, SETBP1, ASXL1 und SRSF2.

CLL / Chronisch lymphatische Leukämie (B-CLL): Prognosemarker IGHV-Status, TP53, SF3B1, NOTCH1

PCR und Sequenzierung: Exons 4-9 von TP53, Exons 12-16 von SF3B1, distale Hälfte Exon 34 von NOTCH1 Die methodisch bedingte Nachweisgrenze für somatische Mutationen kann abhängig von Mutationstyp und B-Zell-Anteil der Probe variieren. In der Regel sind Frameshift-Mutationen bis ~5%, Punktmutationen bis ~10% Signalanteil detektierbar.
IGVH: PCR VDJ, Datenbankabgleich, statistische Auswertung

Indication

B-CLL Patienten mit hohem Progressionsrisiko sind definiert durch mindestens 2 der folgenden Faktoren: Serumthymidinkinase > 10U/l; Lymphozytenverdopplungszeit < 1 Jahr, ungünstige Zytogenetik (17p-, 11q-) oder ungünstiger IgVH Status. Das höchste Progressionsrisiko weisen jedoch Patienten mit Deletion in 17p13.1 auf. 17p13.1 enthält das Tumorsuppressorgen TP53. Hierbei zeigt sich, dass meist das zweite, nicht deletierte Allel von TP53 durch Punktmutationen geschädigt ist. Ein Teil der Patienten mit Normalbefund hinsichtlich Chromosom 17 weist jedoch Punktmutationen als einzige Aberration in TP53 auf. Auch diese Patienten haben ein hohes Progressionsrisiko und sprechen schlecht bis gar nicht auf die Chemotherapie mit Fludarabine an. Andere Therapien, wie z.B. die Behandlung mit Alemtuzumab oder Ibrutinib sind bei Aberrationen von TP53 deutlich wirksamer. Für Patienten mit unauffälligem FISH Befund hinsichtlich Chromosom 17 kann die molekulargenetische Untersuchung der Exons 4-9 von TP53 und ein Ausschluss von Mutationen des Gens prognostisch relevant sein.
Aktuellen Publikationen zufolge gelten neben Mutationen oder Deletionen von TP53 und unmutiertem IGVH-Status auch Mutationen der Gene SF3B1 und NOTCH1 bei CLL als unabhängige, mit verschlechterter Prognose assoziierte, molekulargenetische Marker, die in ca. 17% bzw. 11% der B-CLL nachgewiesen werden (NOTCH1 bei Trisomie 12: 25-28%).1-5 Mutationsträger mit 30-40% OS nach 10 Jahren, daher ggf. relevant bei Entscheidung zur Transplantation).7

Note

Mutationen von NOTCH1 sind bei CLL2 wie auch bei Mantelzelllymphom (12% der MCL!)4 prognostisch ungünstig.6 Der langfristige Erfolg (OS und EFS) einer Transplantation der CLL wird durch SF3B1, TP53 oder NOTCH1 Mutationen nicht negativ beeinflusst.1 Weitere Informationen zum IGHV-Status bei CLL können Sie unserem Informationsblatt IGVH entnehmen.

Indication

Typischerweise finden sich Mutationen des Exons 12 von JAK-2 bei Patienten, die bei Auftreten einer klinischen Symptomatik nur eine isolierte Erythrozytose mit vermindertem Erythropoietin zeigen, während die meisten Patienten mit V617F Mutation auch erhöhte Leuko- und Thrombozytenzahlen aufweisen.

Tyrosinkinaseinhibitoren wie Imatinib, Dasatinib, Nilotinib.
Auch für andere bei CMML bekannte Chromosomenaberrationen werden Therapieerfolge mit Kinaseinhibitoren wie Imatinib (Glivec) berichtet.

Indication

CMML mit Eosinophilie, Abklärung nicht reaktiver Eosinophilien, aCML, CEL, MPN, mit Eosinophilie, selten AML. CMML mit t(5;12)(q33;p13) zeigen meist Eosinophilie. Etwa 2-10% aller CMML sind positiv für die t(5;12)(q33;p13).
Etwa 50% aller PDGFRB Rearrangements entfallen auf die t(5;12)(q33;p13).

Indication

Relevant für Therapiewahl und Transplantationserfolg bei AML.
Das Vorliegen einer Mutation von KIT - meist Exon 17 bei t(8;21)(q22;q22) oder Exon 8 bei Inversion 16 (inv(16)(p13q22) - ist ein zusätzlicher Prognoseparameter der sogenannten "core binding factor"-CBF-AML und dann mit ungünstiger Prognose assoziiert. Prävalenz: Ca. 12% der AML mit t(8;21)(q22;q22) und 10% der AML mit inv(16)(p13.1q22) oder t(16;16)(p13.1q22).

Die ohne zusätzliche Mutation in KIT als günstig zu wertende Translokation t(8;21)(q22;q22) sowie die Inversion 16 inv(16)(p13q22) oder t(16;16)(p13.1q22) betreffen beide den "core binding factor", einen Regulator der normalen Hämatopoese.

T-Zellen bzw. der NK-Zellen der Probe bzw. der Größe einer immunologisch abgrenzbaren Subpopulation mit evtl. pathologischen Oberflächenmarkern. Immunmagnetische Zellanreicherung möglich.

Indication

Etwa 28-40% aller T-LGL und 30% der NK-LGL weisen aktivierende, somatische Mutationen der SH2 Domäne von STAT3 auf (Exon 21).1,2 Ein positives Ergebnis stützt die die Diagnose LGL Leukämie.

Note

Bewertung gemeinsam mit weiteren molekulargenetischen (TCR Klonalität?), morphologischen, immunhämatologischen und klinischen Befunden empfehlenswert.

Indication

Bei etwa 30% der Fälle von SM wird vor, während oder nach ED der SM eine begleitende hämatologische Nicht-Mastzell Neoplasie festgestellt (zuvor: SM-AHNMD, neue WHO: AHN). Im Rahmen der Stufendiagnostik bei V.a. MPN:

DD ET und MF: 1. JAK2_617F, 2. Calreticulin (CALR), 3. MPL, 4. Falls DD isolierte Erythrozytose/PV: HRM Exons 12-15 JAK2

MPL ist als Rezeptor für Thrombopoietin entscheidend an der Regulation der Thrombopoese beteiligt. Gain of function Mutationen, die sowohl erworben, als auch hereditär auftreten können, führen zu einer Thrombozytose und Krankheitsbildern wie der Essentiellen (bzw. Familiären) Thrombozythämie (3-5% MPL-mutationspositiv) oder Myelofibrose (5-8% MPL-mutationspositiv).
Bei ET oder MF ohne Mutation in JAK2 sollen sich Mutationen in MPL bei bis zu 12% der Patienten finden.
Mutationen, die im Zusammenhang mit hereditärer oder erworbener Thrombozytose beschrieben wurden, finden sich in den Exons 2, 3, 4, 10 und 11 sowie in den Introns 10 und 11 von MPL.
Selten führen missense oder nonsense Mutationen von MPL auch zur kongenitalen amegakaryozytären Thrombozytopenie (autosomal rezessiv). Betroffen sind hier alle Bereiche des Gens.

Vergleichsweise höhere Prävalenz: JAK2 und CALR Mutationen. Auf dem ASH im Nov. 2013 erstmals vorgestellt und parallel publiziert: CALR Mutationen treten bei 67-82% der JAK2 negativen ET und bei 88% der JAK2 negativen MF auf (mutually exclusive mit JAK2 V617F!).

Prognostische Markersuche bei histologisch gesicherter primärer oder sekundärer (z.B. post PV) Myelofibrose. Indication

Insbesondere bei zytogenetisch unauffälligem Befund ist der Nachweis somatischer Mutationen diagnostisch für eine klonale Erkrankung sehr sensitiv, z.B. MDS, AML (>50%), CMML (>90%) und schließt - obwohl meist nicht pathognomonisch für eine bestimmte Entität - reaktive Veränderungen aus. Einige Mutationsbefunde können entscheidungs- und/oder therapierelevant sein und lassen sich zur MRD-Diagnostik nutzen.

Neben strukturellen oder numerischen Veränderungen an Chromosomen kennt man heute zahlreiche somatische Gen-Mutationen bei hämatologischen Neoplasien. Ein Mutationsscreening in relevanten Teilen von Genen, die gemäß Literatur bei myeloischen Neoplasien (AML, CMML, MDS, MPN) Mutationen zeigen können, kann in Ergänzung zu (molekular-) zytogenetischen und hämatologischen Untersuchungen aus einer Probe EDTA-/ Heparin-Knochenmark oder Blut erfolgen.

Obwohl meist nicht pathognomonisch für eine bestimmte Entität, entspricht nahezu jeder mutationspositive Befund am ehesten einem klonalen Geschehen, das nicht mehr mit reaktiven oder toxischen Einflüssen zu erklären ist und entscheidungs- und/oder therapierelevant sein kann. Somit ist der diagnostische Nutzen der molekulargenetischen Parameter (31 Loci) gerade dann gegeben, wenn andere diagnostische Verfahren noch ohne klares Ergebnis sind.

Etwa die Hälfte der Patienten mit AA zeigt auch gleichzeitig eine PNH, diese durch PIG Mutationen hervorgerufen. Bei AA Vorhersage des Ansprechen auf immunsuppressive Therapie möglich, günstig: PIGA, BCOR, BCORL1ungünstig: ASXL1, DNMT3A, TP53, RUNX1, JAK2, JAK3, CSMD1; OS-Prognose bei AA günstig: PIGA, BCOR, BCORL11, ungünstig: DNMT3A, ASXL1, TP53, RUNX1, CSMD1; PFS-Prognose bei AA günstig: PIGA, BCOR, BCORL11, ungünstig: DNMT3A, ASXL1, RUNX1, JAK2, JAK3; Übergänge von AA/PNH zu MDS/AML durch klonale Evolution treten bei ca. 15% der Patienten auf und lassen sich oft an Mutationsspektrum und Variantenallelfrequenz beurteilen. 7% der AA und 2.5% der MDS zeigen auch STAT3-positive T-Zell Klone. Mutationen von PIGA sind ursächlich für PNH und führen zu einer beeinträchtigten Synthese von Glycosylphosphatidylinositol Ankermolekülen (sog. GPI Anker). Die Diagnose wird u.a. durch Immunphänotypisierung gesichert. Nur bei atypischen klinischen Manifestationen/atypischen durchflusszytometrischen Befunden kann genetische Diagnosesicherung sinnvoll sein.

Prognostische Bedeutung der Molekulargenetik:

  • sofern ET: Unabhängig von Alter, Leukozytose und Thrombosen25-27 sind Mutationen in EZH2, IDH2, SH2B3, SF3B1, TP53, U2AF1 von erheblicher, prognostischer Relevanz.
  • sofern PV: Unabhängig von Alter, Leukozytose, Venenthrombosen und Karyotyp25-27 sind Mutationen in ASXL1, IDH2, SRSF2 von erheblicher, prognostischer Relevanz für leukämiefreies,- fibrosefreies- und Gesamtüberleben.28,29
  • sofern MF: CALR Status (Typ I [–like] Mutation?) und Anzahl Mutationen in ASXL1, EZH1, IDH1, IDH2, SRSF2 von prognostischer Relevanz, vgl. „MIPSS70“ und „MIPSS70 plus“ Score.25 Für MF ist eine prognostische Einschätzung zu evtl. Transplantation mittels MIPSS70 Index möglich (oder auch „MIPSS70 plus“ Index, inklusive Zytogenetik. Im MIPSS70 Index ab 2 Scorepunkten intermediäres Risiko, ab 5 hohes Risiko. Zur Vervollständigung des MIPSS70 Index erforderlich: Hb, Leukozyten, Thrombozyten, Blastenzahl im pB, konstitutionelle Symptome, Fibrosegrad, CALRTyp1-Status (hier unklar, ob Typ I Mutation). Zur Berechnung online vgl. http://mipss70score.it MIPSS70“ Score 0-1 „LOW“, 2-4 „INTERMEDIATE“, ab 5 „HIGH“; MIPSS70 plus: Score 0-2= “LOW”, 3=”INT”, 4-6=”HIGH”, >7= “VERY HIGH” mit 5-Jahresüberleben zwischen 7% (“very high”) und 91% („low“). Etwa 30% der AML M4 und 20-25% der AML M2 weisen ebenfalls Mutationen des Gens KIT auf. Diese verschlechtern die ansonsten gute Prognose (höheres Rezidiv-Risiko M2+M4, niedrigeres Gesamtüberleben M2). Vorliegende KIT Mutationen können als therapeutische Targets genutzt werden. Hierbei ist die genaue Identifikation der vorliegenden Mutation überaus relevant für die Therapiewahl!

CLL: Stufendiagnostik durch Sequenzierung der Exons 4-9 von TP53
Falls V.a. Li-Fraumeni-Syndrom: Stufendiagnostik durch Sequenzierung der Exons 1-10 und MLPA des Gens TP53.

Indication
  1. CLL: Pathogene Punktmutationen von TP53 sind - genau wie Deletionen von 17p13.1 (TP53-Genregion) - mit einer verschlechterten Prognose assoziiert und finden sich überwiegend bei CLL hemizygot (Kombination aus Punktmutation/Deletion), bei 20-50% der Fälle jedoch auch ohne Deletion (heterozygot) oder compound heterozygot (beide Allele des Gens tragen eine Punktmutation). CLL mit Mutation oder Deletion von TP53 sind Höchstrisiko-CLL mit ungünstiger Prognose und Bedarf für ein adaptiertes Therapieschema. Neuesten Erkenntnissen zufolge zeigen jedoch auch B-CLL mit Deletion/Mutation in 17p13.1 einen recht unterschiedlichen, klinischen Verlauf, Hierbei hängen Prognose und Therapie führend von drei Kriterien ab: RAI-Stadium >1, unmutierter IGVH Status und >25% der Kerne pos. für del17p13.1. Patienten mit Punktmutationen und/oder Deletionen von TP53 sprechen schlechter auf die Chlorambucil/Fludarabin/Rituximab basierte Standardtherapien an, besser dagegen z.B. auf die Therapie mit Alemtuzumab.2,3 Bzgl. möglicher positiver Therapieeffekte4-8 von Dasatinib oder Actinomycin D11 bei Patienten mit TP53-Mutationen oder Deletionen bzw. unmutiertem IgVH-Status bleiben die Ergebnisse klinischer Studien abzuwarten. Weiterhin ist für die therapierefraktäre oder rezidivierte CLL - aber auch für die Erstlinientherapie - als neue Substanz mit vielversprechenden Ergebnissen PCI-32765 (BTK Inhibitor Ibrutinib9,10) zu nennen (Fa. Pharmacyclics, bereits Zulassung für refraktäre CLL, Stand März 2014 named patient program der Fa. Janssen möglich).
  2. Erbliche Disposition im Rahmen eines Li-Fraumeni (like)-Syndroms.
